I am Shivanya Patel and I hereby request you to kindly set an appointment with you. Last Month, You had advised me to come back for another appointment so that, you could ensure my recovery.

I was suffering from a Teeth Cavity and some minor fillings were needed. I am satisfied from your treatment which you had prescribed me and wanted to have the appointment. It would be really nice of you if you could suggest me the date and time to meet for the appointment. It would be more better if the appointment would be possible anytime this week because I am flying to Mumbai for an important interview.
